1. Drivers licenses are required for all applicants.
2. If paying monthly, we require a voided check. No starter checks are accepted. If you do not have checks we will accept a letter printed on your bank’s letterhead with your name, routing number and account information.
3. If your current address is not reflected on your driver’s license, we will need a proof of residency, such as a utility bill or lease agreement.
If a child has a different last name from the parents, we need a copy of their birth certificate.
